While Lord Shiva is the presiding deity, the temple places major significance on Goddess Bhadrakali. The prathishta of Bhadrakali entails a peedom and sword, and pujas devoted to her are carried out twice a day, in the afternoon and in the night, with the night time puja adhering to Shakteya rules (Shakti puja). Mamanikkunnu Sri Mahadevi Temple follows the Shaaktheya tradition of worship, specifically in accordance with the Kaula Marga of Tantra, setting it in the various thirteen important temples in Kerala following this tradition. Upon entering the temple complex, traffic is greeted by the sanctum sanctorum of Lord Shiva facing east, at the same time as the sanctum sanctorum of Mahadevi and Seven Mother Goddesses (Saptha Mathrukkal) facing north is located on the left side. Other deities, consisting of Sastha and Kshetrapala, have separate sanctum sanctorums within the temple.
